House Calls is a not-for-profit program providing frail, vulnerable, marginalized and homebound seniors with physician-led interdisciplinary care at home. 

Eligible Clients
  • Are 65 years or older; 
  • Live in the House Calls catchment area (see map, right); and
  • Have great difficulty accessing a family physician because of physical, cognitive and / or psychiatric disabilities.

The House Calls Team 
Under the leadership of primary care physician Dr. Mark Nowaczynski, the integrated House Calls team includes a Social Worker, Occupational Therapist, Registered Nurse and Nurse Practitioner, supported by a Program Manager and Intake Worker. 

House Calls service is available Monday to Friday, 9:00am to 5:00pm.

No Cost to Clients  
House Calls is supported by the project partners and the Toronto Central LHIN, through Ontario’s ‘Aging at Home’ strategy.
